Roots and Wings Connections Family Counseling LLC has closed, with the final day of clinical services being January 23, 2026.

The practice is no longer scheduling or providing therapy services. This website is maintained for informational purposes only during the post-closure period.

Records Requests

Former clients who wish to request a copy of their records should call the number below or email the provided email address to initiate the request. Once contact is made, a Records Release Authorization Form will be provided via the Simple Practice portal. Upon receipt of a completed authorization, records will be released in accordance with applicable law.
